Diabetes Management Program
The SCFHT Diabetes Management Program provides patient-centered education, screening, diagnosis and treatment of diabetes.
What can we help with?
- Education about day-to-day lifestyle management skills for patients and their family
- Treatment of diabetes aimed at preventing or delaying disease progression and complications
Services and Supports:
- Individualized education, screening and treatment of Type 1 diabetes, Type 2 diabetes and gestational diabetes. Treatments can include: nutrition, weight management, foot care, medications (including insulin starts), blood glucose monitoring, and more
- Referrals for preventative health care
- Diabetes community screening events to screen and identify at-risk community members
Who is it for?
- Patients living with diabetes or at-risk for diabetes
- For patients with or without a primary care provider
- Patients may self-refer, or be referred
